Critical violations: Cheese sauce is not at 135 degrees. It is possible after talking to the employees, the cheese was not reheated completely prior to turning down the holding unit temperature. Items not date marked as needed.

Critical violations: Multiple bottles and containers in prep area without labels. Used wiping cloth stored on prep surface.

El Campesino Supermercado & Restaurant

Critical violations: Open employee canned beverage being kept in meat display cooler. Meat hanging across walk-in cooler on a reused aluminum foil wrapped rod. No date marks on food items in refrigerator or walk-in cooler. Repeat violation. Rods with meat hanging from them not a smooth, cleanable surface.

Noncritical violations: No certified food protection manager on staff. Open boxes of chicken being stored on floor in walk-in cooler. Ice scoop being kept in machine with handle in contact with the ice. Raw meat debris buildup on grinder power button in walk-in cooler.

Critical violations: Ranch dressing at 45 degrees in the salad prep refrigerator.

Noncritical violations: No documentation of a certified food protection manager. Several of the handwashing sinks had no handwashing signage. Buildup of residue on top of ice machine. Several bottles of cleaning products or sanitizers were stored in unlabeled bottles. Facility lacks refrigerator thermometers in some of the refrigerators or cold holding units.

Kwality Ice Cream & Bakery

3404 S.E. Macy Road, Suite 18, Bentonville

Noncritical violations: No certified food safety manager. Three-compartment sink faucet was leaking. Permit expired 8/31/2021.

Critical violations: Sauce at 44 degrees in the prep refrigerator.

Noncritical violations: No documentation of a certified food protection manager. Posted permit is expired.

Critical violations: Handwashing sink blocked by stool and equipment. Items in walk-in and refrigerator not date marked.

Noncritical violations: No certified food protection manager on staff. Roach on back of handwashing sink in kitchen. Pot of beans being kept on floor of kitchen.

Noncritical violations: No documentation of a certified food protection manager. No dish temp plate or heat test strips to measure the heat sanitizer temperature in the dish machine. No retail food permit posted.

Noncritical violations: Facility lacks draining shelving for dishes. Restrooms lack self-closing mechanism. One of the bathrooms lacks a covered trash can. Flooring lacks coving.

Noncritical violations: No test strips. Facility should be deep-cleaned prior to use. Posted permit expired on July 31, 2022.

Noncritical violations: A box with packages of to-go items stored on the floor.

Noncritical violations: No documentation of certified food protection manager. Posted permit expired on April 30, 2020.

2505A S. Walton Blvd., Bentonville

Critical violations: An employee was eating on prep table. Employee went from washing dishes at the dish machine to peeling onions. Biryani in the walk-in cooler was at 45 degrees. Biryani was prepared yesterday evening. Yogurt sauce at 49 degrees and curry sauce at 46 degrees in front cold hold unit. Multiple bottles and containers are not labeled with what they contain. Multiple food containers in the walk-in cooler and in the dry storage area had plastic containers with no handles stored in them.

Noncritical violations: No certified food safety manager. Two freezers have duct tape holding together their door. Surfaces throughout the entire kitchen have an accumulation of grease, food and dust. Some areas to note are: vent above grill area, outside surfaces of prep coolers, shelves in walk-in cooler, outsides of food storage containers (like rice, flour, seasonings, etc.), walls throughout food prep area and handles to all coolers.

Critical violations: Box being kept in handwashing sink in back by dish area and is blocked by mop buckets.

Noncritical violations: Leak in beverage machine filter system in back room. Piece of tile missing in floor near three-compartment sink.

Noncritical violations: Facility lacks refrigerator thermometer in baby room refrigerator.

Critical violations: Sanitizer in three-compartment sink was at 200 ppm chlorine.

Noncritical violations: No chlorine test strips could be located during inspection.

Noncritical violations: Grease buildup on floor under the fryer. Debris on floor under shelving in the back storage area. One of the fans in the freezer is frozen up, and there is an accumulation of ice on the floor, ceiling and on some boxes.

Critical violations: Uncovered employee beverage in prep area. Employee prepping ready-to-eat fruit with bare hands.

Critical violations: Boxes and bags of single-use items (foam cups) on walk-in cooler floor.

Critical violations: No date marking on the Hillshire Farms sandwiches. Removed from shelf. Eggs date marked July 2022. Lunch meat expired 8/10/22 and 8/6/22. Removed from shelf.

Noncritical violations: Floor under milk shelves in walk-in is visibly dirty.

816 E. Main St., Siloam Springs

Critical violations: Raw chorizo stored over beverages.

Noncritical violations: Dried fish not labeled.

Critical violations: Food items in upright cooler were above 41 degrees. New cold hold unit. Ambient temperature showed at 44 degrees so unit was adjusted to a lower temperature.

1402 S.E. Shane Lane, Bentonville

Critical violations: Wipe bucket concentration at 0 ppm chlorine. Food items (sour cream at 50 degrees, diced tomatoes at 49 degrees and cheese at 51 degrees) in front cold hold unit were above 41 degrees.

804 E. Main St., Siloam Springs

Critical violations: Thermometer reading 50 degrees in cooler. Employee said the door was open for restocking. Cheese has a date mark of 2/28/22. Removed from shelf.

Noncritical violations: Permit expired. Permit fee may not be paid.

Critical violations: Hand-drying towels not available at bar handwashing sink.

ABC Happy Kids Learning Center

301 S.E. 28th St., Bentonville

Critical violations: Two unlabeled spray bottles hanging over three-compartment sink.

Noncritical violations: No heat test strips or a maximum registering thermometer available to check dish machine. No chlorine test strips for sanitizer.

3001 N.E. 11th St., Bentonville

Noncritical violations: No documentation of a certified food protection manager. No written procedures for the time as a control. Apples with stickers were out for the breakfast service. Facility approved without three-compartment sink, uses dish machine for sanitizing.

2410 S.W. 14th St., Bentonville

Noncritical violations: No certified food safety manager. Leak in ceiling is causing tile over fridge to bulge due to water buildup.

Critical violations: No sanitizer onsite.
